What Zyrtec is

Zyrtec is the brand name for cetirizine, a second-generation antihistamine. It blocks histamine, the chemical that causes allergy symptoms, at H1 receptors. The item we link to is the 24-Hour Dye-Free Dissolve Tablet, which melts on the tongue without water. Each tablet has the same 10 mg dose as the regular tablets. One dose covers 24 hours.

Unlike first-generation antihistamines such as Benadryl, cetirizine was designed to stay mostly outside the brain. That's why it's usually labeled "non-drowsy" or, more accurately, "less drowsy".

How well it works

Cetirizine is one of the best-studied allergy medicines available without a prescription. A 2026 network meta-analysis for the ARIA guideline panel found that all the major second-generation antihistamines beat placebo for allergic rhinitis. Cetirizine was among the stronger performers, though the review rated most differences between these drugs as small and low-certainty PMID: 41707943. A separate network meta-analysis of randomized trials also found real differences in efficacy between oral antihistamines, so they aren't interchangeable PMID: 37271114.

In a head-to-head outdoor study during pollen season, cetirizine relieved symptoms faster and more completely than loratadine (Claritin) PMID: 8621847. Most people notice relief within about an hour.

What it treats well: sneezing, runny nose, and itchy nose, eyes and throat.

What it doesn't treat well: a blocked, stuffy nose. For congestion, steroid nasal sprays such as Flonase work better than any antihistamine pill PMID: 9848901.

Drowsiness and other side effects

This is Zyrtec's main trade-off. Cetirizine causes drowsiness more often than fexofenadine or loratadine. In a large prescription-monitoring study, cetirizine had a higher rate of reported sedation than loratadine or fexofenadine PMID: 10784544. In a randomized trial, fexofenadine 180 mg matched cetirizine's symptom relief with less drowsiness PMID: 14582814.

Practical tips:

Take your first dose in the evening to see how it affects you.
Don't combine it with alcohol, sleep aids or other sedatives.
Ask a clinician first if you have kidney or liver disease, or are pregnant or breastfeeding.
The FDA has warned about rare cases of severe itching after stopping cetirizine or levocetirizine that was taken daily for months. If you've used it long-term, talk to a clinician before stopping.

Zyrtec vs. generic cetirizine

The drug and the dose are the same, and generics must meet the FDA's bioequivalence standards. Brand Zyrtec typically costs several times as much per dose as generic cetirizine. The dissolve-tab format is the main reason to pay extra: it helps kids 6 and up, and anyone who struggles to swallow pills.

Who should buy it

Buy Zyrtec if you want strong, fast daily relief from sneezing, itching and a runny nose, and you value the no-water dissolve tablet.

Buy generic cetirizine instead if you're happy with regular tablets. It's the same medicine for less.
